About “This” Position
As a CNA/Home Health Aide, you will be responsible for delivering the highest quality care to our patients within the home, in a one-on-one setting. Job responsibilities include but are not limited to:
- Following clients support plan/care plan for tasks
- Recording observations and tasks and signs each entry on the appropriate form for each visit made.
- Communicating on a weekly basis or more often as necessary with the supervisor
- Immediately reporting emergency situations by phone to supervisor or office
- Keeping an accurate timesheet
- Following assignments and regulations
- Reminding clients about their medication intakes
- Planning and preparing meals for the client based on their specific dietary requirements, and if requested, feeding the client
- Assisting with personal care and a variety of daily living activities
- Assisting with home management tasks
- Providing companionship
Qualifications:
- At least 1-yearworking experience in an ALF and/or Home Health setting and a valid CPR, Driver’s license, and Proof of car insurance
- Must have a high school diploma/GED
- Must be able to pass a level 2 background screening
