Residential Care in a home-like environment
Clearstone Group provides long-term Residential Care and Habilitation Care to developmentally disabled adults residing at Bella Group Home in Apopka and at Bella Senior Living in Daytona Beach, rather than their personal or family home. Our clients at both locations can expect the following services.
- 24 /7-supervision and monitoring of residents to ensure their safety at all times.
- Supporting Basic Activities of Daily Living (BADL) such as eating, bathing, showering, grooming, walking, dressing/undressing, transfers, and toileting.
- Supporting Instrumental ADL such as housework, financial management, shopping, preparing meals, communicating with the outside world, and medical management.
- Providing transportation to and from community outings, medical appointments, and other commitments.
- Tracking short and long-term care in accordance with support plans and Individualized Service Plan (ISP).
We work together with all members of client’s care to make sure individualized service is delivered:
- Primary Doctors of residents to check-in about regular visits
- Case Managers of residents to ensure appropriate resources are secured
- Family members and guardians, if available
- Sponsoring Agencies and other specialists, as needed
